When you schedule as a new patient in our office we send you an email with a link to sign up for our patient portal. Our portal allows you access to all forms that must be filled out prior to your child’s appointment. This portal will also allow you to check the date and time of upcoming scheduled appointments as well as watch educational videos.
- Once you receive the invitation you have 24 hours to log in and create a password. At this time you do not need to complete all the forms, you just need to activate the account. If you are not able to get to the emailed invitation within 24 hours, simply email or call the office to have it resent.
- When logging into the portal for the first time you will need the email address and the zip code you have on file with our office and a parent’s date of birth. Depending on how we have your account setup please try both parent’s date of birth if one doesn’t work.

If you need to get into the portal after you have set up your account, you can click below or you can find a link on the home page of our website. Once you are there you will use your email address and password that you created to log in. If you don’t remember your password you can click the link to reset it.
