When you schedule as a new patient in our office we send you an email with a link to fill out forms that must be filled out prior to your child’s appointment. This portal will also allow you to check the date and time of upcoming scheduled appointments as well as watch educational videos.
- When logging into the portal for the first time you will need to enter the parent’s date of birth. Depending on how we have your account setup please try both parent’s date of birth if one doesn’t work.
